Medical Director role for an FP/IM in Tampa, FL

Chronic Care in Home program in Florida is looking for a Family Practice or Internal Medicine physician to become a Medical Director. 

• Board Certified or Board Eligible Family Practice or Internal Medicine Physician
• Medical Director role - all admin/operational - no patients unless no providers available
• Will need to be based out of Tampa, FL and will oversee the Palm Beach, Dade, and Orlando counties as well
• Mon-Fri - 40 hours a week
• National well established Primary Care private practice group with a Special Care at Home program
• Program focus is to keep patients out of the hospital - clinic to home model - long term disabled, palliative care, hospice, or chronically ill patients
• NP's and MA's go into homes and see patients
• Must have experience as Medical Director
