NP/PA opportunity for Annual Health Assessments in Salem, OR

Great opportunity in Salem, OR for an NP/PA to perform Primary Care in a clinic and Annual Wellness Visits in-home working for a private group. 

• Family Nurse Practitioner (FNP) or Adult Nurse Practitioner (ANP) or Physician Assistant (PA)
• 2+ yrs exp as NP required - Primary Care, Family Med, Internal Med, Gerontology, or Palliative Care exp. Home Care exp is a plus
• Duties will include working in a clinic and performing annual wellness exams for senior Medicare patients in their home
• This role is a mix of outpatient in-clinic setting, in-home health visits, and occasional telehealth appointments
• Collaborate with Primary Care team to ensure completion of holistic patient care
• Must have active OR medical and drivers license, active DEA, currently enrolled in Medicare and in good standing, have HCC coding exp, and have Annual Wellness Visits experience
• 40 hours a week - Mon-Fri 8a-5p
• Salary based on experience, mileage reimbursement and includes plenty of great benefits
